Important CSLB changes – New applications & Fees

Last year CSLB created a new version of many of its applications.  As of May 1st, 2017, ONLY the new version is being accepted. If a previous version application is submitted, it will be rejected and will need to re-submitted with the fees. This will greatly delay the processing time of your application!

Below is a list of the forms that have been updated:

  • Application for Original Contractor License (13A-1)
  • Application for Original Contractor License – Examination Waiver (7065) (13A-1a)
  • Section 4 – Personnel Full Legal Names and Addresses (13A-1d)
  • Request for License Number Reissuance (13A-1h)
  • Application for Additional Classification (13A-2)
  • Application for Replacing the Qualifying Individual (13A-2a)
  • Application for Joint Venture Contractor License (13A-3)
  • Application to Inactivate Contractor’s License (13A-5)
  • Application to Add New Personnel to an Existing Corporate or Limited Liability Company License (13A-7)
  • Application to Report Change of Title for Current Officer or Personnel of Existing Corporate or Limited Liability Company License (13A-7a)
  • Certification of Work Experience (13A-11)
  • Application for Asbestos Certification (13A-25)
  • Application for Hazardous Substance Removal and Remedial Actions Certification (13A-26)
  • Application to Add a New Limited Partner to an Existing Partnership License (13A-62)
  • Owner-Builder B-General Building Construction Project Experience (13A-64a)
  • Application to Change Home Improvement Salesperson Address (13H-8)
  • Application for Registration as a Home Improvement Salesperson (13H-11)
  • Application to Change Contractor Business Name or Address (13L-4)
